Annual Report Submission Format
Format to fill and submit Annual Report. It is advised to do so every January and submit copy to the MAWB for their records and retain one copy with you during inspection time.

Form III - Format for Maintenance of Records (Breeder)
Breeder will need to maintain Form III at Breeding Establishment which has details about the procurement/acquisition/purchase of where he or she bought the dog/s from. Form 3 will be checked during inspections.
Form IV - Format for Maintenance of Records (Breeder)
Breeder will need to maintain Form IV at Breeding Establishment which has details about the Sale carried out and details to whom he or she has sold the puppies/dogs. Form 4 will be checked during inspections.
Form V - Format for Maintenance of Records (Breeder)
Breeder will need to maintain Form V at Breeding Establishment which has details about the health, medical and breeding records of the animals in the establishment. Form 5 will be checked during inspections.
Exercise Checklist - Format for Maintenance of Records (Breeder)
Breeder will need to maintain Exercise Checklist at Breeding Establishment which has the exercise record of all individual animals with the breeder. Exercise Checklist will be checked during inspections.
